The Ky Boiler Permit Application form is the Boiler Installation Permit Application of the Kentucky Department of Housing, Buildings and Construction, Boiler Section (500 Mero St, Frankfort), for installing boilers and related pressure piping in Kentucky. It asks for the license holder and company name and mailing address, the boiler contractor's phone numbers and Kentucky license number with expiration date, where the unit is installed, and the business owner or manager there. The applicant gives the quantity of each unit, such as high pressure steam boilers, economizers, low pressure heating or domestic boilers, hot oil boilers or kitchen steamers, plus the start date, job number and an email address for the permit letter. The fee is set by a range based on the project's value, with the check payable to the Kentucky State Treasurer. If more than 100ft of piping is installed, a piping permit is also required.
