How to fill out Form SU 87-65, box by box
The person receiving the vehicle
Write the full name of the person receiving the vehicle, the address (street and number, apt or floor #), the city/town, state and zip code, and the date and year on which you did receive and accept the vehicle as a gift or grant.
The vehicle
Enter the year, make (e.g. Chevrolet) and model (e.g. Camaro) of the vehicle, and its vehicle identification number (VIN).
The person gifting the vehicle and the relationship
Write the full name of the person gifting the vehicle, with the address (street and number, apt or floor #), the city/town, state and zip code.
Write the relationship of the person who gifted you the vehicle.
Signing under penalty of perjury
You swear that no monies, services, barter arrangements, or other articles or things of value or consideration in any form were paid or given in connection with or relating to the gift of this vehicle. If subsequent information discloses that the statements contained herein are false, then all penalties imposed by law will be evoked.
Write the day (number), the month and the year on which it is signed under penalty of perjury.
The person receiving the gift (the grantee) signs on the line marked GRANTEE'S SIGNATURE.

Before you go
A notarized gift letter, in place of a Bill of Sale, must accompany the affidavit.
