Duplicate Title Affidavit
The Duplicate Title Affidavit must be signed by the owner and notarized.
Check lost, mutilated or never received for the certificate of title issued to you.
You make application for a duplicate copy of said certificate and agree to hold the Commissioner harmless if the previous title is obtained by another person.
To give the Commissioner permission to mail the title to the address on this application, check the box.

You have and will maintain, during this registration period, vehicle liability insurance (security) required by LRS Title 32:861-865. Failure to maintain as agreed will be a violation of law which may result in criminal prosecution and/or suspension of registration privileges.
If the vehicle being registered is defined as a commercial motor vehicle by the Federal Motor Carrier Safety Regulations and/or Federal Hazardous Material Regulations, the registrant declares knowledge of those federal regulations by signing.

Affidavit of Non-Possession of Title by Lienholder
The Affidavit of Non-Possession of Title by Lienholder must be signed by the lienholder and notarized.
The lienholder swears or affirms that title of the vehicle described on the form, showing lien in its favor, was never received or was received and surrendered to the owner. Check never received, or received and surrendered to the owner.
The Lienholder's Signature must be signed by lienholder and notarized: the two Witness lines are for the witnesses, and the Sworn and subscribed before me this day of line and the Notary Public Signature, Printed Name and ID Number are for the Notary Public.
Plates, stickers, title correction, farm use and out of state declaration
A title, license plate, or validation sticker is not received
If a title, license plate, or validation sticker is not received within thirty (30) days after submitting application and fees for such, contact your nearest Office of Motor Vehicles. Failure to notify the Office of Motor Vehicles timely could result in additional fees being charged.
Departmental policy provides for the issuance of a free replacement within sixty (60) days from the date of issuance.
A. Conversion
For a conversion, enter the Old License Plate Number, Use and Weight, and the New License Plate Number, Use and Weight.
B. Transfer of plate
Enter the License plate that has been removed from a vehicle and transferred to the vehicle described on the first page of this form, the Year, Make and VIN of the vehicle it was removed from, and the License plate returned.
C. Lost, stolen, or replacement plate sticker
Check Plate or Sticker for a lost, stolen, or replacement, and enter the License Plate Number.
D. Title correction
Enter the Error and what it is Correct To.
E. Farm use statement
You declare that you are a bona fide farmer and are in the business of farming; that the vehicle described on the first page of this form is used primarily, but not exclusively, in hauling farm produce raised on your farm, from such farm to market and return therefrom; hauling merchandise to your farm and that this vehicle will not be used to haul another person's farm produce or other products for a fee or compensation. Enter where the farm is located, with the RFD, Box, City, Parish and Zip, and sign the Owner's Signature line.
F. Out of state declaration
Enter the Printed name of owner who declares, and the Make, Model and VIN that was domiciled in Louisiana on the line you fill in next.
