How to fill out Form CS-053, box by box
Company Name (including trade names) and Business Address (location to be licensed)
Enter the Company Name (including trade names).
Enter the Business Address (location to be licensed), with City, State and Zip Code.
Name and type of storage location
For a storage location, enter the Name and type of storage location, with Street Address, City, State and Zip Code.
Type of Business (check appropriate blocks)
Under Type of Business, check the appropriate blocks.
Under Dealer Licenses, check Wholesale, Title Service, New Vehicle, Emergency Vehicle, Used Vehicle, Manufacturer, Trailer, Distributor or Motorcycle.
Next to ADR, enter the # of Acres.
Next to Scrap Processor, enter the # of acres.
Under Transporters, check Inspection Station, Vehicle Painting/Remodeling/Repair, Auctioneer, New Vehicles for Manufacturer or Other, and write the Other on the line.
Check Driver Education, DIP or RSDEP, and Classroom or Business Office.
Zoning official certification
The zoning official writes the business after "I certify, that the business of" and marks does or does not meet all zoning requirements, including the issuance of a use and occupancy permit, if required.
The zoning official signs at SIGN and enters the Printed Name, Date, Official Capacity, Telephone Number and Email Address.
