What it is: The Odometer Disclosure Statement: Federal and State law requires that you state the mileage in connection with the transfer of ownership.
Who signs: The seller signs at Seller's Signature and enters the Seller's Printed Name. The buyer signs at Buyer's Signature and enters the Buyer's Printed Name.

How to fill out Form VR-197, box by box
Mileage and discrepancy statements
After "I," print the seller's name; the seller certifies to the best of that knowledge that the odometer reading is the actual mileage of the vehicle unless one of the following statements is checked.
Enter the miles, with no tenths. The odometer reading is the actual mileage unless one is checked: "The mileage stated is in excess of its mechanical limits" or "The odometer reading is not the actual mileage. Warning - odometer discrepancy".
The vehicle
Enter the Make, the Model and the Year.
Enter the Body Type and the Vehicle Identification No.
The seller
Enter the Seller's Street Address, then the City, the State and the Zip Code.
Enter the Date of Statement.
The buyer
Enter the Buyer's Street Address, then the City, the State and the Zip Code.
