Filing: the Affidavit cannot be filed more than 1 year after the alleged violation was discovered or reasonably should have been discovered.
Notice: the aggrieved party attests to mailing a certified return receipt requested notice to the Respondent's last known address, allowing at least 10 business days.
One per page: list only one alleged violation on page 2 and include the applicable labeled attachments (exhibits) behind each.
