Enter the Year, Make, Model, Vehicle identification number and Body style.
Odometer reading and the mileage to check
Enter the odometer reading. Please enter digits exactly as shown in your vehicle. No tenths.
Check one box after "and certify that, to the best of my knowledge, the odometer mileage is": actual mileage; not actual mileage (Warning: odometer discrepancy); exceeds mechanical limits of odometer (five-digit odometer has rolled over). Under the *Note for the *Check one boxes: mileage cannot be corrected at a later date. Do not estimate mileage.
Seller's information
Enter the Name of seller(s) (please print), the Address, and the City, State and ZIP code.
Sign on the line for Signature(s) of seller(s) and enter the Date.
Purchaser's information
Enter the Name of purchaser(s) (please print), the Address, the City, and the State and ZIP code.
Purchaser(s), aware of the seller(s) certification, sign on the line for Signature(s) of purchaser(s) and enter the Date.
Leased vehicles only: Lessee's information
Leased vehicles only: enter the Name of lessee(s) (please print), the Address, and the City, State and ZIP code.
Sign on the line for Signature(s) of lessee(s) and enter the Date.
Leased vehicles only: Lessor's information
Leased vehicles only: enter the Name of lessor (please print), the Address, and the City, State and ZIP code.
Enter the Date odometer statement sent to lessee and the Date received from lessee.
Enter the Name of lessor's agent (please print), sign on the line for Signature of agent and enter the Date.
If your situation is different
The vehicle is leased
Use the reverse side for leased vehicles.
Federal law and Section 233a of the Michigan Vehicle Code require that a lessee disclose the mileage to the lessor when transferring ownership of a leased vehicle.
