The LCL Packing Declaration (December 2024 Version) is a statement about the packaging used in a consignment. It must be issued by the packer of the goods on company letterhead that includes the company's name and physical address, and it gives the vessel name, voyage number and consignment identifier or numerical link. Question 1 asks whether unacceptable packaging materials, such as straw, peat, hay, chaff or used fruit and vegetable cartons, were used as packaging or dunnage. Question 2 asks whether timber or bamboo packaging or dunnage, such as crates, cases, pallets or skids, was used. If so, Question 3 certifies whether it was treated and marked in compliance with ISPM 15 (timber only), treated in compliance with Department of Agriculture, Fisheries and Forestry treatment requirements with a treatment certificate, or not treated. A company employee signs with a printed name and the date of issue.
