NAVMC 10476 at a glance
What it is: NAVMC 10476, the Reserve Qualification Summary (RQS), is intended to provide boards with an update of military and civilian skills and qualifications which may not be reflected in the Official Military Personnel Files (OMPF).
Where and when to file: Upon completion, submit to the appropriate Headquarters element as required.
All officers, staff noncommissioned officers, and sergeants of the Selected Marine Corps Reserve, Individual Ready Reserve, Active Reserve, Mobilization Training Units, and Individual Mobilization Augmentees may complete the form, when required.

How to fill out NAVMC 10476, box by box
1. Name, 2. EDIPI, 3. Age, 4. Current Rank and 5. DOR
Type the service member's last name, first name and middle initial.
Enter the service member's EDIPI, which is located on the back of the Military ID (CAC) card.
Disclosure of the EDIPI is mandatory, and the EDIPI is used for purposes of individual identification only.
Enter the service member's current age, current rank and date of rank.
6. PMOS, 7. AMOS, 8. AMOS2 and 9. Current Military Duty
Enter the service member's primary military occupational specialty (PMOS), and an additional military occupational specialty (AMOS) in AMOS and in AMOS2.
Enter the service member's billet, unit and component organization (ex: Policy Analyst/Reserve Affairs, M&RA).
10. Primary Residence, 11. Phone and 12. Email
Enter the service member's home address (city, state, zip).
Enter the service member's home, cell and work phone numbers.
Enter the service member's personal or work e-mail address.
13. Languages, 14. Height, 15. Weight, 16. Body Fat, 17. PFT and 18. CFT
Enter the service member's foreign language spoken / written/ reading ability.
Enter the service member's height and weight that correspond with the most current PFT or CFT (box 17 or 18).
Enter the service member's current body fat (if applicable).
Enter the service member's most current PFT date and PFT score, and most current CFT date and CFT Score. These scores can be found on Marine-On-Line.
19. Current Civilian Occupation
Enter the service member's current civilian occupation, job title and description of duties.
20. Civilian Education
Enter the service member's current and/or concurrent education level and vocational or special skill licensing or certification. Include the educational institution and the date diploma or certification was completed.
21. Military Education
Enter the service member's military schools attended. Include completion date.
22. Military Career Experience Summary
Enter the service member's career experience summary. Include the billet(s) held, the unit and whether the billet was in the active or reserve component. Include the dates served in those billets.
23. Decorations
Enter the service member's award(s) to include personal award(s), campaign medal(s), unit award(s), etc. Include the date the award was received.
24. Remarks
Enter any miscellaneous information that pertains to the duties and responsibilities of current or future military billets. This can include, but is not limited to, community activities, civilian skills beneficial to the military, hobbies, philanthropy, sports, etc.
25. Signature and 26. Date Completed
Type in the service member's signature.
Enter the date the RQS was completed.

Disclosure is voluntary; however, failure to provide the information would preclude up to date information regarding skills and qualifications from being available in official case files, and may hamper chances for future military assignment and promotions.

Updating OMPF information
This form can be submitted as part of an individual's package submission to boards, but will not be used to update OMPF information.
Marines should conduct a separate audit of their records to ensure that all of their skills and qualifications are accurately reflected in the Marine Corps manpower system.
