How to fill out Form TC 96-192, box by box
The affiant and the secured party
Enter the county after STATE OF KENTUCKY, COUNTY OF, then the name of the affiant, the affiant's Title (President, Secretary, etc.) and the Name of Secured Party.
The security agreement, the debtor and the vehicle
Enter, in the day of 20 line, the day on which the debtor entered into a security agreement with said secured party, the Name of Debtor, the Make of vehicle, the Type of Vehicle (passenger car, trailer, etc) and the Serial or Identification No.
VIN, Year, Model, Body Style and Odometer reading
Enter the VIN No., the Year, the Model and the Body Style.
Enter the Odometer reading; an odometer reading is required on all transfers.
Titled on and the File No.
Enter the date on which the financing therefore was titled on, 20 and the File No., with the county in the County Court Clerk's office, State of Kentucky.
Why the vehicle was taken and the covenants violated
Since that date, possession of the vehicle has been lawfully taken by the secured party for the reason that the debtor has defaulted in the payment of one or more of the cash installments provided for in the security agreement, and/or the debtor has committed a breach of one or more of the covenants in the security agreement; list here any state covenants violated, if any.
Disposed of to a purchaser or retained by the secured party, and the last Kentucky license
The vehicle described on this form has been disposed of to the Name of purchaser (enter the Address of Purchaser too) as provided in KRS 355.9-503 and 355.9-504, or has been retained by the Secured Party (enter it) under KRS 355.9.505 in satisfaction of the debtor's obligation. Notice has been given to the debtor and all other persons entitled thereto pursuant to the application section. The last Kentucky license number issued for said vehicle was No., County of, for year of.
