Prior to death: this form, along with the title, must be completed, signed and submitted to the MVA prior to the death of the vehicle owner.
Titled in Maryland: vehicle must be currently titled in Maryland.
Ownership: the beneficiary designation does not affect the ownership of a vehicle until the death of the sole owner or the last surviving co-owner.
Lien: if a lien exists, it must be satisfied prior to being retitled or accompany a letter from the lienholder indicating the lien has been transferred.
Trust: if the beneficiary is a trust, a copy of the trust must be submitted when re-titling the vehicle.
