7. Acknowledgment by the notary public
   The bottom of the form is the notary’s acknowledgment. The form must be notarized, so ensure that it is signed in the presence of a notary public.^[1] The notary will fill out their section, confirming your identity and signature.^[1] The notary enters your name on the “personally appeared” line, the date, and signs on the Notary Public line.
