If you have lost your debit or credit card, or if it has been stolen, you can use this form to request the release of the relevant bank account details. The form must be completed in full and submitted to your nearest Absa branch. Once the form has been processed, you will receive notification from Absa indicating whether the release of information has been approved or not. Blocked bank account details can be a major inconvenience, so make sure to submit your Ab 92 form as soon as possible if you need access to them.
