DAF Form 899 (20221021), Request and Authorization for Permanent Change of Station, Military, is the Department of the Air Force form that directs a service member to proceed on a permanent change of station (PCS); it is prescribed by DAFMAN 36-2102. The form records the member's rank, name and SSN, specialty code and security clearance, the unit relieved from and the unit to be assigned, reporting instructions, travel days authorized by privately owned conveyance, any temporary duty (TDY) en route, the type and length of tour, dependent travel and dependents, join-spouse details, and the authority, PCS code and accounts the PCS and TDY expenses are charged to. Its remarks tell the member to submit a travel voucher within 5 workdays after travel and, under AFI 32-6000, to report to the base housing referral office at the new duty station before entering any rental, lease or purchase agreement for off-base housing.
