How to fill out Form MVT 5-9, box by box
Vehicle identification number, year, make and model
The owner completes Part I, Owner/Lienholder & Vehicle Information: enter the vehicle identification number, serial number or hull identification number, then the year, model, make, color and cylinders.
Owners and addresses
Enter Owner 1 Name and Owner 2 Name, then the Resident Physical Address and the Mailing Address.
Lienholder and lien date
Enter the Lienholder Name, the Lien Date and the Lienholder Address.
Appointment of Licensing Official by Owner
The owner completes Part II, Appointment of Licensing Official by Owner: write the Licensing Official and the County, Alabama, appointed as attorney-in-fact to sign the owner's name and do all things necessary to apply for a title on the above vehicle, manufactured home or vessel.
Appointment of Deputy by a Licensing Official
The licensing official completes Part III, Appointment of Deputy by a Licensing Official: the licensing official appoints the government official in Part IV as deputy for the physical inspection as provided under Chapter 8 of Title 32, with the Licensing Official's Name (Printed).
Vehicle Inspection by a Government Official
The government official completes Part IV, Vehicle Inspection by a Government Official, certifying completed physical inspection of the above referenced motor vehicle, manufactured home or vessel and that the information on the vehicle matches the information listed on this form.
Enter the Government Official's Name (Printed) and the Government Official's Agency Name.
