What Is This Employee Appraisal Form?
This document is the official performance evaluation form for Alabama state employees. Issued by the Alabama State Personnel Department and last revised in January 2006, it is a 2-page form that supervisors complete at the end of each review period to assess employee performance, work habits, and disciplinary record. Who Needs to Complete This Form?
The form is completed by supervisors for employees in Alabama state government positions. Both parties must sign after the appraisal conference has taken place. How to Fill Out the Alabama State Employee Appraisal
- Enter employee information. Write the employee's full name, social security number, job classification, class code, and the performance review period at the top of the form.
- Rate each work habit category. Score attendance, punctuality, cooperation with coworkers, and rule compliance in the work habits section.
- Calculate the responsibility score. Sum all responsibility ratings across the performance categories listed on the form.
- Record any disciplinary actions. If disciplinary events occurred during the review period, document them in the designated section. Subtract the disciplinary score from the responsibility score to determine the final appraisal score.
- Support extreme ratings. If the rating is "Does Not Meet" or "Consistently Exceeds" standards, attach supporting documentation and file a copy in the agency's personnel records.
- Conduct the appraisal conference. Review the completed document with the employee before the agency deadline.
- Collect signatures. Both the employee and supervisor sign the form. The employee's signature confirms receipt, not agreement.
