The B6T Application for Private School Transportation is issued by the New Jersey State Department of Education's Office of Student Transportation. Parents must file a separate form for each child. The form must be submitted by the annual deadline set by the school district, and late submissions are handled on a case-by-case basis.
Students in grades K through 8 attending private or religious schools in New Jersey are eligible to apply. For students entering kindergarten or enrolling for the first time in grades 1 through 8, a copy of the birth certificate is required. The form collects the student's full name, date of birth, gender, home address, commuting distance to school, and attendance hours. It also includes the name and address of the private school and the name of the last school attended. A parent or guardian must sign the form to certify that all information is correct.
