Step 1: Open the form. Click the orange button at the top of this page. The BSA Exemption Review Form will open in the FormsPal editor.
Step 2: Enter institution and customer details. Type the depository institution name and the customer's legal business name. Add the tax identification number, business address, and account number. These fields identify the parties to the exemption.
Step 3: Select the exemption category. Choose whether the exempt person is a listed company, a listed company subsidiary, or a non-listed business. For non-listed businesses, the account must have been open for at least two months before the exemption applies.
Step 4: Document the basis for exemption. Record the nature of the business and the average monthly volume of cash transactions. The Bank Secrecy Act requires businesses to regularly exceed $10,000 in cash transactions to qualify as non-listed exempt persons.
Step 5: Complete the annual review. Federal law requires banks to review each exemption at least once per calendar year. Confirm the business activity has not changed. An authorized bank officer must sign and date the completed review.
