A reviewing official evaluates the driver's history of infractions, accidents, and compliance with safety requirements, including hazardous materials handling. The official then determines whether the driver meets minimum safety standards or is disqualified under Section 391.15. Both the driver and the reviewer must sign the form. Employers are required to keep the completed document in the driver qualification file for three years from the date of signing. Employees who hold a commercial license should also maintain a daily vehicle inspection report as part of their safety program.
