The Chinese Visa Application Form V.2013 is the official form required for all foreign nationals applying for a visa to mainland China. It is issued by the Embassy of the People's Republic of China and must be completed in block letters in English or Chinese. Every field is mandatory unless otherwise indicated on the form.
Before you begin, collect the following: your current valid passport (valid for at least six months beyond your planned stay), a blank visa page in your passport, one recent passport photo meeting official specifications (48mm x 33mm, white background, taken within the last six months), a confirmed travel itinerary, hotel booking confirmations for your full stay, and proof that you can cover travel costs.
The form is divided into four parts. Part 1 collects personal information, including your full name as shown in your passport, sex, date of birth, place of birth, current and all former nationalities, national ID number, religion, and marital status. Passport details are also recorded here: passport type, number, date of issue, date of expiry, and issuing authority. Part 2 covers your occupation, employer or school name and address, educational background, and contact phone number. Part 3 collects your home address, email address, emergency contact details, and information about your immediate family, including your spouse's name, nationality, and occupation. Part 4 is the travel section, covering purpose of visit, number of entries, intended dates of travel, port of entry, accommodation address in China, and the identity of whoever is funding your trip.
The form closes with a health and security declaration. You must answer questions about any communicable diseases, mental or physical conditions, or criminal records. You must also list all countries you have visited in the past 12 months. Your signature at the end confirms that all information provided is truthful, complete, and accurate.
Visa requirements and supporting documents vary by type. Tourist visa (type L) applicants need a confirmed round-trip flight itinerary and hotel bookings. Business visa (type M) applicants need an invitation letter from a Chinese business entity. Student visa (type X) applicants must provide an acceptance notice from a Chinese educational institution. Standard processing at most consulates takes 4 business days for the first application.
