Step 2: Complete Part 1 - Personal Information. Enter your family name and given name exactly as they appear in your current passport. Provide your sex, date of birth, and place of birth. Enter your current nationality and any former nationalities you have held. Add your national ID number and religion where indicated. Under Passport Details, enter your passport type (ordinary, diplomatic, official, or other), passport number, date of issue, date of expiry, and the full name of the issuing authority.
Step 3: Complete Part 2 - Present Occupation. Select your occupation category: business person, government official, military, student, or other. Enter your current job title and the full name and address of your employer or educational institution. Select your highest education level from the list. Fill in your contact phone number and ZIP code in the designated fields.
Step 4: Complete Part 3 - Family Members and Contact Information. Enter your full home address including ZIP code, your home phone number, mobile number, and email address. Under the family information fields, enter your spouse's full name, nationality, and occupation. Add your emergency contact's name, relationship to you, and mobile phone number. Include your parents' names and nationalities as required by the form.
Review your home address and email address fields carefully at this point, as errors in these fields are the most common reason for consulate follow-up requests.
Step 5: Complete Part 4 - Travel Information. Select your purpose of visit from the options listed: tourism, business, family reunion, study, employment, transit, or other. Enter the number of entries requested (single, double, or multiple). Provide your planned dates of entry and exit, your intended port of first entry, and your complete accommodation address in China, including the hotel name and street address. Identify who will fund your trip (yourself, your employer, your school, or another sponsor). In the travel history section, list every country or region you have visited in the past 12 months.
Step 6: Answer the health and legal declaration questions at the end of the form. Each question requires a yes or no answer. The questions cover communicable diseases, physical or mental health conditions, past involvement in terrorist activities, and any criminal convictions in any country. Answer all questions honestly, as providing false information can result in visa rejection and future entry bans.
Step 7: Review all fields for accuracy. Click "Done" to finalize your form. Download the completed PDF and print it on white A4 or US Letter paper at 100% scale. Sign the form in the signature box using a black or blue pen. Attach your passport photo (48mm x 33mm, white background) with a paper clip, not staples. Submit the signed form together with your passport, photo, and all required supporting documents to the nearest Chinese embassy or consulate office.
