Statement Guide is for: Title or Salvage Title Established by Surety Bond (refer to DR 2922); In Lieu of Bond for Motor Vehicle 25 Years Old or Older (refer to DR 2462); In Lieu of Bond for Manufactured Home 25 Years Old or Older (refer to DR 2463); In Lieu of Bond for Trailers 2000 lbs or Less (refer to DR 2908).
Vehicle Description
Under Vehicle Description, enter the Vehicle Identification Number (VIN), Year, Make, Body, Model, Color and C W T.
Enter the Fuel Type. The * after Fuel Type applies: if electric, mark Yes or No for plug-in electric.
Off-Highway Vehicle and Snowmobile
Mark Yes or No for Off-Highway Vehicle and for Snowmobile.
Dealer Number, Date Purchased, Commercial Use and Bus Capacity
Enter the Dealer Number and the Date Purchased.
Mark Yes or No for Commercial Use, and enter M S R P and Size (Width x Length).
Bus Capacity: mark Adult or Juvenile.
Applicant's Name and address
Enter the Applicant's Name, Street Address, City, State and ZIP Code.
Reason Title Not Available
Enter the Reason Title Not Available. If more space is needed, please provide on a separate sheet.
Vehicle purchased from
Enter who the vehicle was purchased from.
Enter the Date Purchased and the Price, and mark Yes or No for Bill of Sale Attached.
Efforts to have the seller establish title
State your efforts to have the seller establish title.
Attempts to contact the owner of record
State your attempts to contact owner of record to insure they have no interest in the vehicle.
Present condition of the vehicle and liens
Mark the present condition of the vehicle: Roadworthy or Not Roadworthy.
Mark Yes or No under Are there any LIENS against this vehicle? Enter the Name of Lienholder.
Signature and date
Enter the Signature and the Date (MM/DD/YY).
