• Missing or incorrect data in Repair Parts and Special Tool Lists (RPSTL)
• Discrepancies in Supply Catalogs or Supply Manuals (SC/SM)
• Opportunities to improve clarity, accuracy, or safety in any Army document

Key Sections of the Form

1. From: Enter your name, grade, and organization.
2. To: Address to the proponent office listed in the publication.
3. Publication Data: Record the publication title, number, date, and the specific page or paragraph you are recommending a change to.
4. Remarks: Clearly describe your recommended change, referencing the exact section, table, or figure by number.
5. Signature Block: Sign and date before forwarding to the proponent.
