In GRADE, enter the grade of the individual, in pencil entry only.

Size and authorized allowance

In SIZE, enter required sizes at time of initial entry, in pencil entry only, and periodically confirm or revise as necessary.

In AUTHORIZED ALLOWANCE, enter the authorized allowance, in pencil entry only.

Entries in ink: Name, Rank, DOD ID Number, Quantity, Date and Signature; columns 1 thru 15 are used for recording consecutive clothing inventories.
