DA Form 5515 is the Training Ammunition Control Document (TACD) used by the United States Army to record the issuance and return of training ammunition and explosives during field exercises and live-fire training events. Governed by Army Regulation AR 710-2 (Supply Policy Below the National Level), with DCSLOG as the proponent agency, the form establishes complete accountability for every round distributed to a unit.
The document records the document number, issue and turn-in dates, and specifics for each ammunition type.
