What Is DA Form 3644?

DA Form 3644, the Monthly Abstract of Issues of Petroleum Products and Operating Supplies, records the movement of fuel and operating supplies at Army installations. The form covers all petroleum products issued during a reporting month. It is part of the Army property accountability system, governed by DA PAM 710-2-1.

The Deputy Chief of Staff, G-4, serves as the proponent agency for this form. Any Army post, camp, or station that issues petroleum products must complete DA Form 3644 at the end of each month.
