What Is DA Form 3644?

DA Form 3644, the Monthly Abstract of Issues of Petroleum Products and Operating Supplies, records the movement of fuel and operating supplies at Army installations. The form covers all petroleum products issued during a reporting month. It is part of the Army property accountability system, governed by DA PAM 710-2-1.

The Deputy Chief of Staff, G-4, serves as the proponent agency for this form. Any Army post, camp, or station that issues petroleum products must complete DA Form 3644 at the end of each month.

The form tracks five data points for each product listed:

• Product type, grade, and unit of issue - identifies the specific item being tracked
• Opening balance - quantity on hand at the start of the reporting period
• Quantities received - total received from each supply source during the month
• Quantities issued - total given out to each unit or activity during the month
• Closing balance - quantity remaining at the end of the month

Some petroleum products must be converted to gallons for the monthly totals. DA PAM 710-2-1 provides the conversion tables for products tracked in other units of measure. Products covered include motor gasoline, diesel fuel, jet propellant JP-8, lubricating oils, hydraulic fluids, and antifreeze compounds.

The accountable property officer signs the completed form to certify that all entries are correct. Completed forms go into the monthly supply accountability package and are used to post totals to stock record accounts. Commanders review the data to track fuel consumption trends and identify shortages before they affect mission readiness.

Accurate and timely completion of DA Form 3644 supports audits, unit inspections, and Inspector General reviews. Errors on this form can delay property book closings and require time-consuming corrections to accounting records.
