The DA Form 5704-R is the official US Army Alternate Pistol Qualification Course Scorecard used to assess and record a soldier's marksmanship performance on the alternate pistol course. The form organizes the qualification process into numbered firing tables, each covering a specific shooting position and distance requirement. Positions tested include standing, kneeling, crouching, and prone, both with support and unsupported. Additional tables evaluate performance under CBRN (chemical, biological, radiological, and nuclear) conditions and during nighttime firing.

The form also includes procedural sections covering mandatory magazine changes, loading and unloading requirements, and CBRN masking drills. These sections ensure that all safety and regulatory standards are observed during the qualification course. The completed and signed scorecard serves as the official record of the soldier's pistol qualification and is filed with the unit.
