How to fill out DA Form 7680-R, box by box

Name, Unit and Date

Enter the NAME (First, Last, MI), the UNIT and the DATE (YYYYMMDD).

Zero, Record Fire, Total Points, Rating Scale and Initials

After the grenadier has zeroed the weapon, he records the zero data for future reference under ZERO, LEAF SIGHT, DEFILADE and ELEVATION.

In the RECORD FIRE, mark HIT or MISS for each TGT.

For TOTAL POINTS, award 10 points for each hit.

On the RATING SCALE, TOTAL POINTS (Block 7) of 80 - 90 is EXPERT, 70 - 75 is FIRST CLASS, 60 - 65 is SECOND CLASS and 0 - 55 is UNQUALIFIED.

Enter the GRADER'S INITIALS with its DATE (YYYYMMDD) and the OIC'S INITIALS with its DATE (YYYYMMDD).

Skills Test: Name and Date

Before performing live fire, soldiers take the 40-mm grenade launcher skills test. This exercise is used to determine the grenadier's proficiency on dry-fire tasks associated with the M320 40-mm grenade launcher.

Enter the NAME and the DATE (YYYYMMDD).

Stations 1 to 5, Grader's Initials, OIC's Initials and Date

At STATION 1 (maintain an M320 GL), mark GO or NO-GO for: 1. Cleared the M320. 2. Disassembled the M320. 3. Inspected the M320. 4. Reassembled the M320. 5. Performed a function check.

At STATION 2 (install mounting brackets, leaf sight assembly, and DNS), mark GO or NO-GO for: 1. Installed mounting brackets. 2. Installed leaf sight. 3. Installed DNS.

At STATION 3 (load, unload, and place M320 on safe), mark GO or NO-GO for: 1. Loaded. 2. Unloaded. 3. Placed on safe.

At STATION 4 (correct malfunction on M320), mark GO or NO-GO for: 1. Kept the weapon pointed at the target. 2. Waited 30 seconds then opened the breach. 3. Removed the round. 4. Examined the primer.

At STATION 5 (ID ammo), mark GO or NO-GO for: 1. Identified the M781 TP round. 2. Identified the M406 HE round. 3. Identified the M433 HE round. 4. Identified the M585 star cluster round. 5. Identified the M583 (WHITE), M661 (GREEN), and M662 (RED) star parachute rounds. 6. Identified the M713 (RED), M715 (GREEN), and M716 (YELLOW) ground smoke marker rounds. 7. Identified the M992 infrared illuminant round. 8. Identified the M1006 nonlethal cartridge. 9. Identified the M1029 crowd dispersal cartridge.

Commanders may add to, but not delete from, tasks on the skills test. Once basic proficiency has been attained, commanders can include special conditions, such as CBRN or limited visibility.
