If you are unable to locate either the MCO, MSO or Certificate of Title, you must complete an Application for Confirmation of Conversion (Form 5314) and provide it to the director of revenue.
Ownership document: Place your initials in the box that applies to the type of ownership document currently in your possession.
MCO or MSO: The original MCO or MSO must be surrendered to the Director of Revenue with Form 5315.
Lease: If you have initialed the box for possession of the real estate under a lease, the Lessor must complete the Consent of Lessor section.
