What it is: Form BA-8 (Revised 07/26) is the NJMVC ENTITY IDENTIFICATION NUMBER (EIN) REQUEST FORM.

What to bring and attach to Form BA-8
Initial (Complete A): submit this completed application with the following. 1. A copy of the following business organization documents: a. A certified document from the IRS displaying the entity's name, current address (same as the Address in Section A) and FEIN; b. NJ Certificate of Formation or NJ Certificate of Incorporation from the Division of Commercial Recording; or NJ Business Registration/Trade Name Certificate; 2. If Executive applying for MVC EIN is not listed on any of the corporate documents an original or certified copy of power of attorney; and 3. If the Executive requesting MVC EIN holds an out-of-State I.D., a color photocopy of the driver's license or other photo identification. 4. Lienholders only - submit items 1a, 2, and 3 and one of the following documents accordingly. a. Banks - Copy of Banking Commission Documents b. Credit Unions - Copy of Charter Papers Note: All other financial institutions and out-of-state dealerships - no other documents are required.

Initial, Address Change or Name Change
Initial (Complete A), Address Change (Complete A&B), Name Change (Complete A&C).
Section A. Business Information
In Section A. Business Information, enter the Legal Name of Company and the Federal Employer Identification Number (FEIN).
Under Mailing Address on file with the Internal Revenue Service (IRS), enter the Address, City, State and Zip.
Enter the Business Description.
Executive requesting MVC EIN
Enter the Name of Executive Requesting MVC EIN and the Title.
Enter the Relationship to Company, the Driver's License Number and the State.
Signature, Date, Tel. No., Fax No. and Email
Sign on the Signature line and enter the Date.
Enter the Tel. No., the Fax No. and the Email.
Section B. Change of Business Address
In Section B. Change of Business Address, enter the NJMVC Issued EIN, the Old Address with its City, State and Zip, and the New Address with its City, State and Zip.
Section C. Change of Legal Business Name
In Section C. Change of Legal Business Name, enter the NJMVC Issued EIN, the Old Business Name and the Amended Business Name.

Section C. Change of Legal Business Name
Section C. Change of Legal Business Name: submit the following. 1. Amended NJ Certificate of Formation or NJ Certificate of Incorporation from the Division of Commercial Recording; or NJ Business Registration/Trade Name Certificate; 2. A certified document from the IRS displaying the amended business name (same as the Amended Business Name in Section C), current address and FEIN; 3. If Executive applying for MVC EIN is not listed on any of the corporate documents an original or certified copy of power of attorney; 4. A color photocopy of the driver's license or other photo identification of the requestor.
Federal Employer Identification Number has changed
IMPORTANT: If the Federal Employer Identification Number has changed due to a Change of Business Address or a Change of Legal Business Name, you must apply for a new NJMVC issued EIN.
