Enter 1. Name (Last, First, MI), 2. Rank/Rate and 3. EMPLID.

4. a. Enter the Permanent Duty Station (PDS) name/address/zip code. b. If you are a Reserve Component (RC) member, enter your Primary Residence /address/zip code.

In Box 5, check the block(s) that apply and indicate the effective date in the space provided. If you have an approved BAH protection authorization from CG PSC-PSD-FS or COMDT (CG-1332) for your PDS assignment, or an approved Family Stability Act BAH/Housing Worksheet (CG-7220H), attach a copy to this worksheet. a. Reporting to permanent duty station: 1. Shore-based duty or 2. Sea Duty. See Note*. b. Assigned to CG/DOD owned/leased: 1. Family-type quarters or 2. Single-type quarters (Unaccompanied Personnel Housing (UPH), shipboard quarters, or family-type housing converted to UPH). See Note*.

Note*: A member without dependents residing in government quarters at their duty station when not required by overnight duty is not eligible for BAH or OHA without dependents or BAH with-dependents based on the payment of child support, even if paying for a residence but does not commute to and from the residence on a daily basis. Members in this situation are deemed to be occupying government quarters and are only authorized the BAH Partial rate, or if paying child support, the BAH Differential (BAH DIFF) rate.

c. terminated assignment from CG/DOD owned single-type quarters, includes family-type housing converted to UPH. Authorized to reside in private sector housing. d. terminated private sector housing and assigned to and occupying CG/DOD owned or leased single-type quarters. e. terminated assignment from CG/DOD owned/leased family-type quarters. Box 6, dependency status (check the applicable block). 6a: Married; With dependents. Spouse is not an active duty military member.

b. Married to an active duty military member and no dependent(s); Without dependents. Attach the Member-Married-to-Member BAH Worksheet (CG-2025B).

c. Married to an active duty member; with dependents. You claim the eligible dependent(s) for BAH. Attach CG-2025B.

d. Married to an active duty military member; Without dependents. Your spouse claims the eligible dependent(s) for BAH. Attach CG-2025B.

e and f: Married to an active duty military member. No dependents from the marriage but you pay child support. e. The custodial parent of the child/children is not an active duty military member. f. The custodial parent of the child/children is an active duty military member. See the Note. Attach CG-2025B.

g. Without dependents.

h. Not married; With dependents. I am the full-time legal and physical custodial parent of my dependent child/children.

i. Not married. I have a CG PPC(LGL) approved illegitimate child/children based on my payment of child support.

j and k: Without dependents and paying child support. j. Your former spouse is not an active duty military member. k. Your former spouse is an active duty military member and has custody of the child/children. See the Note.

l. Not married but I have a CG PPC(LGL) approved Parent, Parent-in-law, Parent in Loco Parentis, Stepparent, Parent by Adoption.

Note: A member is not authorized BAH/OHA with dependents based solely on the basis of the member's child support payment when their child/children is/are in the custody of an active duty military member (including a former spouse) who is either assigned to CG or DOD owned or leased family-type quarters, or is in receipt of a with-dependent housing allowance on behalf of the child/children. The member paying child support is authorized a without dependent housing allowance.

If you are a member without dependents and assigned to sea duty, do not enter a private address if you are assigned to or occupy shipboard quarters for any other reason than for required by duty, or if the address is for a residence that you do not commute to and from your cutter's home port on a daily basis when not on duty or underway; in these cases, enter your cutter's mailing address.

Do not enter a private residence address if assigned to sea duty, without dependents, and occupying shipboard quarters.

9. BAH/OHA start authorization request is for: OHA Note: In non-BAH payable locations, the payment of the Overseas Housing Allowance (OHA) requires a lease agreement or verifiable purchase price. Refer to the DOD Financial Management Regulation, Volume 7A, Chapter 26, section 2606.

a. BAH/OHA With Dependents.

Block 9 BAH Zip Code. If starting BAH with or BAH without, enter the correct BAH zip code. To start BAH Partial or BAH DIFF the SPO/P&A Technician will enter the applicable BAH code.

10. Member's remarks (if needed): enter your remarks.

11. Command Certification. a. The start of BAH is effective and authorized on the date entered in Block 5, and on the zip code indicated in Block 9.

b. The start of OHA requires a completed DD Form 2367, Individual Overseas Housing Allowance (OHA).

12. Member's Signature: sign and enter the Date.

13. Command Approval Signature: sign and enter the Date.
