How to fill out Form CN 17, box by box
REQUEST
Enter the Designated operator of origin and the REQUEST Date.
Mark For withdrawal from the post, For alteration or correction of address or For cancellation or alteration of the COD amount.
Office of origin and Office of destination
Enter the Office of origin with Our fax No., and the Office of destination with its Fax No.
Nature of item
Enter the Nature of item, No. of item, Date of dispatch and Office of origin.
If a facsimile is attached, mark Facsimile attached, and enter the Description (form, colour of the item, etc.).
Sender and Addressee
Enter the Sender (name and full address) and the Addressee (name and full address).
Enter the Original COD amount in figures (where applicable).
Withdrawal from the post
Under Withdrawal from the post, Please return the item: mark priority/air, S.A.L. or non-priority/surface.
Alteration or correction of address
Under Please redirect the item, mark priority/air, S.A.L. or non-priority/surface, and enter the New address or requested alteration.
Cancellation/ Alteration of the COD amount
Mark Please cancel the COD amount or Please alter the COD amount, and enter the New COD amount (in words and figures).
Signature
Sign under Signature.
Particulars to be supplied by the office of exchange
Particulars to be supplied by the office of exchange.
Enter the Date and No., and mark S.A.L., Non-priority/surface or Priority/air.
Mail in which the item was sent abroad: to be supplied for parcels and registered and insured items only.
Enter the Dispatching office of exchange, the Office of exchange of destination and the No. of the bill/list.
Mark Letter bill (CN 31 or CN 32), Special list (CN 33) or Special list (CN 16), and enter the Serial No.
Mark Bulk advice, enter the Date, and sign under Date and signature.
Reply of the office of destination
Under Reply of the office of destination, mark The item in question has already been delivered to the addressee, The request was not explicit enough; please send additional details, The item in question has been seized under the country's internal legislation, or The search was unsuccessful.
Enter the Additional information and the Place and date, and sign under Place, date and signature.
To be returned, duly completed, to the office of origin.
