Name in full, birth, citizenship and addresses

Enter the Date (mm-dd-yyyy) and Place of Birth, and the Evidence of U.S. Citizenship.

Date of death and place of death
Enter the Date of death as Month, Day, Hour, Minute and Year, and the Place of death as Number and street, or Hospital/ hotel, City and Country.
Cause of death
For the Cause of death, include the authority for the statement; if physician, include full name and official title, if any.
Disposition of the remains and of the effects
Enter the Disposition of the remains, and what the Local law governing disinterment of remains provides.
Enter the Disposition of the effects, and the Person or official responsible for custody of effects and accounting therefore.
Traveling/residing abroad with relatives or friends
Under Traveling/residing abroad with relatives or friends, enter the NAME and ADDRESS.
Informed by telegram or telephone
Under Informed by telegram or telephone, enter the NAME, the ADDRESS and the DATE (mm-dd-yyyy) NOTIFIED.
Copy of this report sent to
Under Copy of this report sent to, enter the NAME, the ADDRESS and the DATE (mm-dd-yyyy) SENT.

Notification or copy sent to Federal Agencies lists SSA, VA, OPM, Other and State Agency.
