Proof of ownership: if purchased new, a Manufacturer's Statement or Certificate of Origin; if purchased used, usually a certificate of title.
Proof of name: only proofs of name that contain the owner's signature will be accepted.
Co-owner: if there is a co-owner, both signatures are required.
Liens: Section 4 on page 1 can only be used by a dealer to file a new lien.
Damage disclosure: must be completed for vehicles, motorcycles and trailers.
