How to fill out Form PS-05, box by box
Use this form only if this is the first custody order for these children
Use this form only if this is the first custody order for these children. To change an order, use form PS-07. Do not use this form in a child support case filed by Alabama DHR.
In the Court of (county)
Fill in the county of the Court, then Your Name (Plaintiff in this case) and the Defendant's First and Last Name (the other parent).
Mother's information
Enter the Mother's Name (first name, middle name, last name), Age, Address (street address, city, state, zip) and Phone #.
Is the mother of these children married to the father of these children?
Mark Yes or No: is the mother of these children married to the father of these children?
Father's information
Enter the Father's Name (first name, middle name, last name), Age, Address (street address, city, state, zip) and Phone #. If the person asking for custody is NOT the mother or father of the children, fill out the Name (first name, middle name, last name), Age, Address (street address, city, state, zip), Phone # and Your relationship to the children (explain).
Information about the children
Print the Name and Date of birth of each child/ren under 19 involved in this case.
Who do the child/ren live with now?
Mark who the child/ren live with now: Mother, Father or Other (name and relationship).
Current Address
Enter the Current Address (street address, city, county, state, zip) and how long the child/ren have been at the Current Address.
Previous addresses
If less than 6 months, list as Previous addresses (street address, city, county, state, zip) where else the child/ren lived during the last 12 months.
Other cases involving these child/ren
Mark Yes or No: have there been any other court cases, like child support, juvenile, or DHR about this child/ren? If Yes, enter the Date, County and State where the orders were made, and what kind of orders the court made.
