Request: borrower name, project name, borrower ID, project number, amount requested, date requested and current reserve balance.

Budget: whether this item was identified in the current approved fiscal year budget.

Capital needs: whether this request is part of an approved capital needs assessment.

Identity of interest: whether a company with an identity of interest will perform any of the work; if Yes, identify the company.

Reserve: if the amount of reserve funds used is more or less than the amount approved by Rural Development, we will advise the Rural Development Servicing Official.
