Form T-22B (Revised 5-2020), Certification of Inspection by a Duly Constituted Georgia Law Enforcement Officer, is issued by the Georgia Department of Revenue, Motor Vehicle Division. A Georgia law enforcement officer uses it to certify the inspection of a motor vehicle, motorcycle or scooter, and the vehicle owner then submits it to the county tag office. Section A records the vehicle: year, make, model, body style and color, the public vehicle identification number (PVIN) and how its plate or label is attached, the federal motor vehicle safety label, and the documents presented to establish ownership. Section B records the owner's full legal name, driver's license number, address and telephone. Section C is the officer's certification, with the response to the GCIC/NCIC stolen vehicle inquiry, signed under the penalty of false swearing (O.C.G.A. 16-10-71). The form must be completed in its entirety in blue or black ink; any correction or alteration voids it.
