How to fill out Form T-22R, box by box
A To request an inspection of a Rebuilt Motor Vehicle (choose one), Private Inspector or State Inspector
To request an inspection of a Rebuilt Motor Vehicle, choose one: Private Inspector or State Inspector.
Private Inspector: Generally completed same day. Take this form to the inspection appointment.
Private Inspector: If this vehicle has an out-of-state rebuilt title, the title may be applied for at your local county tag office instead of mailing the documents to the Motor Vehicle Division.
State Inspector: Takes 4-6 weeks to process. Inspections are scheduled during the hours of 8:00 am to 4:30 pm Monday through Friday, excluding state holidays.
State Inspector: State inspector will call contact person to schedule inspection once this form is received with payment and all applicable documentation.
B VEHICLE INFORMATION
Under B VEHICLE INFORMATION, enter the Year & Make of Vehicle and the Vehicle Identification Number (VIN).
C OWNER INFORMATION
Under C OWNER INFORMATION, enter the Vehicle Owner's Name(s), the Street Address and the City, State, Zip.
D VEHICLE LOCATION
Under D VEHICLE LOCATION, enter the Vehicle Location (Residence or Business Name), the Street Address and the City, State, Zip.
E GEORGIA TAX NUMBERS, Sales Tax ID Number and Withholding Tax Number
Under E GEORGIA TAX NUMBERS, enter the Sales Tax ID Number (if applicable) and the Withholding Tax Number (if applicable).
F CONTACT INFORMATION
Under F CONTACT INFORMATION, enter the Work Phone, the Cell Phone and the Person to contact Regarding Inspection.
H OWNERS SIGNATURE
Under H OWNERS SIGNATURE, sign on Signature: and enter the Date.

How much does a salvage inspection cost?
Private Inspector: Be prepared to pay the inspector $100 inspection fee (checked October 20, 2026). I understand that a $100 inspection fee must be paid each time the vehicle is inspected or re-inspected (checked October 20, 2026). Fees due to the Department of Revenue: $18 title fee if vehicle passed private inspection; $118 if requesting state inspector (including title fee) (checked October 20, 2026).
Where do I file it?
If using a state inspector, mail this completed form to: DOR/Motor Vehicle Division, Attn: Salvage Unit, P. O. Box 740384, Atlanta, Georgia 30374-0384. If using a private inspector, submit this completed form to the inspector at the time of inspection.
