How to fill out Form ITD 3403, box by box

The VINs and the VIN location
Write the 1st Vehicle Identification Number (VINs typically have 17 numbers) in the boxes, and use VIN Discrepancy (Explain) to explain any VIN discrepancy.
Write the 2nd Vehicle Identification Number (VIN) in the boxes, then check where the VIN is located: Dash, Door, Headstock, Trailer Tongue L - R, or Other (Describe) and describe it.
Type of Vehicle
Under Type of Vehicle, check one of Auto/PU/Van, Truck, Trailer, Motorcycle, RV, Mobile Home, Snowmobile, ATV or UTV; for an OHV write the Engine Size cc, and for a Motorcycle check FMVSS Sticker Yes or No.
For an ATV or UTV, write the Wheel base (See back/bottom of form) and the Number of Wheels; wheelbase is the distance between the center of the front axle and the center of the farthest rear axle.
Model Year Make Body Type Model Color Fuel Type
Write the Model Year, Make, Body Type, Model, Color.
Write the Length (trailers), Width (trailers, ATVs, UTVs), Weight (Utility and boat trailers, ATVs, UTVs) and Fuel Type.
For a Motorcycle, check Off-highway or On-highway.
Write Comments/Description/Other Pertinent Data.
Odometer, the applicant and the officer's signature
Write the Odometer Reading (no tenths), and mark the Odometer Status, if known: Actual, Not Actual-Warning - Odometer Discrepancy, Exempt, Exceeds Mechanical Limits, or No Odometer.
The Inspection Officer or Agent signs (I certify I have physically inspected the vehicle described above).
Write the Applicant's Name and Applicant's Address, and the Agency Name, Badge Number, City/Area, Date Inspected, City, State, Zip and Daytime Phone Number.

This form must be filled out by a law enforcement officer or an authorized agent of the Idaho Transportation Department (ITD) with data directly from the vehicle without the use of other documentation.
