Age: the claimant is over eighteen (18) years of age, and qualifies as a witness in the State of Idaho.

Successor: the claimant is the successor, as that term is defined in Idaho Code § 15-1-201(5), of the decedent's estate.

Thirty days: the claimant says that more than thirty (30) days have elapsed since the death of the decedent.

Entitlement: the claimant is entitled to payment or delivery of the property of the decedent.

Perjury: the claimant certifies under penalty of perjury pursuant to the law of the State of Idaho that the foregoing is true and correct.
