Signature: the information must be signed by the disabled individual or guardian.
Doctor's statement: the State of Kansas no longer requires a doctor's statement to re-certify continued eligibility.
Where to submit: this form must be submitted to your local County Treasurer's office in person, by mail or email.
Do not send: do not send this form to the Kansas Department of Revenue, Division of Vehicles or processing will be delayed.
