Site Type and Option Type
Select Site Type: New, Existing or Expansion of Existing Site.
Select Option Type: Contractor Optioned or Department Optioned.
Legal Description
Under Legal Description, enter the County, Section, Township Number and Range.
Enter the Site Legal Location (quarter section), Approximate Depth of Excavation and Source Name.
Material/Activity (Check all that apply)
Material/Activity (Check all that apply): Aggregate, Staging Area or Plant Site, Borrow, Stockpile Area, Rock Pile/Rip-Rap, Waste Site.
Project Information
Under Project Information, enter the Name (Project Engineer, Project Manager or NDDOT Tech Support) and the Division/District/Consulting Engineering Firm.
Under Project Information, enter the Project Number or Project Location, the Project Control Number and the Email Address.
Applicant and Comments
Under Applicant, enter the Contact Name and Company Name.
Under Applicant, enter the Telephone Number and Email Address.
