Vehicle/Boat/Manufactured Home Information
All information (except signature) must be typed. Verify the vehicle, hull or manufactured home ID number entered. If information is not legible, is incorrect, or not complete, the lien is NOT recorded. For more information about filing a lien electronically, visit https://dmv.ny.gov/forms/mv-909.pdf
Enter Your DMV Charge Account Number and the Identification Number. Mark This is a: Vehicle, Boat, Trailer or Manufactured home.
Enter the Year, the Make, the Body Type/Hull and the Registration/Plate No. of Borrower, if any.
Owner's Name, Co-Owner's Name and Current Mailing Address
The lien will be recorded only if the name(s) listed as the owner(s) is EXACTLY the same as the owner(s) recorded, or to be recorded, on the Certificate of Title. If a Certificate of Title has not been issued to this borrower, enter the name as it appears on the driver license or non-driver ID card.
Enter the Owner's Name (Last, First, Middle or Business Name) and the Owner's NYS License/Permit/Non-Driver ID Card Number, then the Co-Owner's Name (Last, First, Middle or Business Name) and the Co-Owner's NYS License/Permit/Non-Driver ID Card Number.
Enter the Current Mailing Address (Street Number and Name), the Apt. Number, the City, the State and the Zip Code.
Check the box if the address you entered is NEW since your last title was issued. Your address will only be updated if this box is checked.
In the OWNER'S STATEMENT, the owner understands that the lienholder will send this notice to the DEPARTMENT OF MOTOR VEHICLES. If a title was previously issued in the owner's name for this vehicle, boat, trailer or manufactured home, the owner gave it to the lienholder to be sent to DMV with this notice. The owner understands that a new Certificate of Title that shows the lienholder's name is mailed to the owner at the address on record unless the owner has indicated a NEW address in the box to check for a NEW address.
If signing for a corporation, print your name and title.
Lien Information
Enter the Lien Filing Code only if a code has been assigned to you or your company; DMV assigns the code.
Enter the Lienholder's Name, the Lienholder's Name (continued), the Mailing Address, the City, the State and the Zip Code.
This notice authorizes the Department of Motor Vehicles to disclose (or otherwise make available) information about the lienholder obtained by the department in connection with this record.
Enter the Date of Security Agreement. The Lienholder's Signature must be an original signature or a facsimile stamp.
Mark YES or NO: Has a NY Certificate of Title been issued to this borrower? IF YES, ATTACH THE TITLE.

LIENHOLDER: Mail this form and the owner's title (if issued) to Title Bureau, NYS Department of Motor Vehicles, PO Box 2604, Albany NY 12220-0604. You can go to dmv.ny.gov/titlestat to verify whether a lien was recorded or a title certificate was issued.
