NGB Form 36-11 at a glance
What it is: NGB Form 36-11, the Statement of Understanding for Excess/Overgrade Condition, is used to document the placement of an Air National Guard member into an excess or overgrade condition.
Who signs: The form has three SIGN lines; the first two have a DATE and the last has the DATE OF MILPDS UPDATE. The form does not say who signs each line.

How to fill out NGB Form 36-11, box by box
Section I. Individual identification
Under Section I. INDIVIDUAL IDENTIFICATION, enter NAME, SSN, RANK and UNIT.
Disclosure is voluntary; however, failure to provide your social security number may result in delayed notification of the assignment process.
Section II Excess or overgrade assignment information
Under Section II EXCESS OR OVERGRADE ASSIGNMENT INFORMATION, enter the DAFSC, the UMD RANK and the POSITION NUMBER.
Enter the EXCESS OR OVERGRADE CODE and the APPLICABLE RULE IAW AFI36-2110 Table 17.2/17.3.
Enter the EFFECTIVE DATE and the EXPIRATION DATE.
Section III. Initial the applicable condition code block
The member must initial the applicable condition code block.
EXCESS: I understand that prior to my expiration date of the EXCESS condition, my unit and I share the responsibility to locate and place me in a valid position (within the AFS for which I am currently qualified) as the sole incumbent commensurate with my current grade.
OVERGRADE: I understand that prior to my expiration date of the OVERGRADE condition, my unit and I share the responsibility to locate and place me in a valid position (within the AFS for which I am currently qualified) commensurate with my current grade.
INCENTIVE: I understand that if I am serving in an incentive obligation period, and a valid vacancy is not available in the duty AFSC for which I received the incentive prior to my expiration date (if an extension to my expiration date is not granted) I will forfeit all incentives and may face recoupment of the unearned portion I have received.
IV. Certification
By signing under IV. CERTIFICATION you certify that you were briefed concerning your newly assigned Condition Code (Excess and/or Overgrade Code) and were counseled by your unit commander concerning the significance of this code, its expiration date and, as a consequence, the impact it can potentially have on your Air National Guard career.
