Signature: all challenges submitted must be legible and signed and dated on page 4 by the applicant or they will be returned.
FBI appeal: a separate appeal may also be filed with the FBI, NICS Section.
Complete challenge: pages 1 through 4 must be submitted for a challenge to be considered complete.
