Vehicle Identification Number (VIN)

As an authorized agent or authorized employee, certify that the owner of the vehicle with the VIN listed has authorized you to receive the title on his/her behalf and that you have received this certificate of title from the DMV, and enter the Vehicle Identification Number (VIN).

Authorized agent or authorized employee

Under "I am an authorized agent for:", enter the Vehicle Owner's Name and List relationship to Vehicle Owner, OR under "I am an authorized employee for:", enter the Name of Company or Individual.

Vehicle owner and third party: names, signatures and dates

For an authorized agent or authorized employee, the Vehicle Owner enters the Printed Name of Vehicle Owner, Signature of Vehicle Owner and Date; the Third Party Receiving Title enters the Printed Name of Third Party Receiving Title, Signature of Third Party Receiving Title and Date.

Is there a penalty?

Under SC Code of Laws, Section 56-3-390, any person who fraudulently uses or gives a false or fictitious name or address in any application required to be made under this chapter except for registration, knowingly makes a false statement in any such application or knowingly conceals a material fact in any such application shall be guilty of a misdemeanor and shall upon conviction be punished as herein provided. Any person who shall operate or any owner who shall permit the operation or movement of any vehicle registered and licensed under a violation of this section shall be guilty of a misdemeanor and, upon conviction, shall be punished as herein provided.
