Standard Form 1442: Solicitation, Offer, and Award (Construction, Alteration, or Repair)

In Box 2, mark the type of solicitation: SEALED BID (IFB) INVITATION FOR BID or NEGOTIATED (RFP) REQUEST FOR PROPOSAL.

Enter the solicitation number in Box 1, the contract number in Box 4, the requisition/purchase request number in Box 5 and the project number in Box 6.

In Box 3, enter the date issued and the page of pages.

Enter the issuer and the CODE in Box 7, ISSUED BY.

Enter the ADDRESS OFFER TO in Box 8.

For information call, enter the name and the telephone number (include area code) in Box 9. No collect calls.

In Box 10, THE GOVERNMENT REQUIRES PERFORMANCE OF THE WORK DESCRIBED IN THESE DOCUMENTS: enter the title, identifying number and date.

In Box 11, enter the calendar days within which the contractor shall begin performance and the calendar days within which it shall complete it after receiving the award or the notice to proceed. Mark this performance period mandatory or negotiable.

In Box 12a, mark YES or NO: the contractor must furnish any required performance and payment bonds. If YES, indicate in Item 12b within how many calendar days after award.

In Box 13a, sealed offers in original and the entered copies to perform the work required are due at the place specified in Item 8 by the entered hour, local time, and date. If this is a sealed bid solicitation, offers will be publicly opened at that time.

Sealed envelopes containing offers shall be marked to show the offeror's name and address, the solicitation number, and the date and time offers are due.

In Box 13b, mark that an offer guarantee is or is not required.

In Box 13c, all offers are subject to the (1) work requirements, and (2) other provisions and clauses incorporated in the solicitation in full text or by reference.

In Box 13d, enter the calendar days for Government acceptance after the date offers are due. Offers providing less than that number will not be considered and will be rejected.

In Box 14, enter the name and address of the offeror, including the ZIP Code, and the CODE and FACILITY CODE.

In Box 15, enter the telephone number, including the area code.

In Box 16, enter the remittance address only if it is different than Item 14.

In Box 17, the offeror agrees to perform the work required at the prices specified below in AMOUNTS, in strict accordance with the terms of this solicitation, if this offer is accepted within the entered calendar days after the date offers are due.

Insert any number equal to or greater than the minimum requirement stated in Item 13d. Failure to insert any number means the offeror accepts the minimum in Item 13d.

By Box 18, the offeror agrees to furnish any required performance and payment bonds.

In Box 19, give the AMENDMENT NUMBER and DATE of each amendment to the solicitation whose receipt the offeror acknowledges.

In Box 20b, the SIGNATURE is entered, and in Box 20c the OFFER DATE.

The AWARD section is to be completed by the Government.

The contracting officer will complete Item 28 or 29 as applicable.

The contracting officer types or prints the name in 31a, signs for the UNITED STATES OF AMERICA in 31b and enters the date in 31c.

In Item 28, NEGOTIATED AGREEMENT, the contractor is required to sign this document and return copies to issuing office. Contractor agrees to furnish and deliver all items or perform all work requirements identified on this form and any continuation sheets for the consideration stated in this contract.

Type or print the name and title of the contractor or person authorized to sign in 30a, then sign in 30b and enter the date in 30c.

The rights and obligations of the parties to this contract shall be governed by (a) this contract award, (b) the solicitation, and (c) the clauses, representations, certifications, and specifications incorporated by reference in or attached to this contract.

In Item 29, AWARD, the contractor is not required to sign this document. The offer on this solicitation is hereby accepted as to the items listed. This award consummates the contract, which consists of (a) the Government solicitation and the offer, and (b) this contract award. No further contractual document is necessary.
