For (f) Possession of the Premises located at, fill in the line.

Under 2. Stay of Execution was ordered pending, enter (a) the Money Judgment payable in installments of $ beginning on a Date until paid, (b) Vacating Premises by a Date and (c) Other.

In the DECLARATION OF APPLICANT, under A., fill in $ for what Defendant(s) have paid. For Small Claims, a partial satisfaction of judgment, for the amount paid, must be filed. Please use Judicial Council Form EJ-100. Under B., fill in the Date Defendant(s) defaulted on, by failing to Make scheduled payment or Vacate Premises.
