Form VP-015 at a glance
What it is: Form VP-015 is the Vehicle Inspection Certificate for Vehicle Identification Number Verification under NRS 482.220.
Who signs: The Authorized Officer-Inspector signs the Signature line and enters the Date.
An Authorized Nevada DMV Agent or a Peace Officer completes the form.

How to fill out Form VP-015, box by box
Year, Make, Model and Body Type
For Moped Inspections use form VP-30. Please print or type. The certification "I certify that I have examined the following vehicle" is followed by the Year, Make, Model and Body Type to enter.
Federal Certification Safety Label and Emissions Label (LSVs only)
Mark Yes or No for the Federal Certification Safety Label and Emissions Label (LSVs only).
Vehicle Identification Number
Enter the Vehicle Identification Number.
Odometer Reading
Enter the Odometer Reading (as shown on apparatus): NO TENTHS. If the vehicle's odometer apparatus only displays five numbers, put an X in the first box. Not all LSVs will have an odometer.
WARNING ODOMETER DISCREPANCY
Box 1: The mileage stated is in excess of its mechanical limits. Box 2: The odometer reading is not the actual mileage. WARNING - ODOMETER DISCREPANCY. Box 3: Exempt - Model year over 20 years old.
Trailer Length, Motorhome Length and Number of Axles
If the vehicle being inspected is a trailer, mark Yes or No: does it include living quarters? Enter the Trailer Length in Feet and Inches, the Motorhome Length in Feet and Inches and the Number of Axles.
Fuel Type and REMARKS
Under Fuel Type, mark Gas, Diesel, Propane, Electric or Other (see remarks).
Enter the REMARKS.
Authorized Officer-Inspector
Enter the Authorized Officer-Inspector's Printed Name and Badge or ID #.
The Authorized Officer-Inspector signs the Signature line and enters the Date.
Signatures must be originals. Photocopies are not acceptable.
If other than DMV Inspection Station, Enforcement Agency and Nevada Authorized Agency
If other than DMV Inspection Station, enter the Address, City, State and Zip Code.
Enforcement Agency: enter the Agency.
Nevada Authorized Agency: enter the Agency.
