A motor vehicle power of attorney involves two people. The principal is the vehicle owner who gives the power, and the agent is the person who acts for the owner. The POA document should clearly state what the agent is allowed to do. This can range from handling registration and title documents to selling or buying a vehicle.
All New Jersey POA forms must be signed by the principal and notarized to be valid.[1] This process verifies the principal’s identity and confirms they willingly sign the document.
An agent can take care of vehicle paperwork when you cannot be there in person, which saves a trip and keeps the process moving. These are some common situations where owners sign a power of attorney.

Principal and agent
On page 1, enter your name and address, then the name and address of the agent you appoint to act for you.
The motor vehicle
Enter the Make, Model, Year, VIN Number and Odometer reading. Use Other Identifying Characteristics for anything else that helps identify the vehicle, such as its color.
Social Security Number/Tax ID Number and date of birth
Still on page 1, fill in your Social Security Number or Tax ID Number and your date of birth. The effective date statement printed below these lines has nothing to enter.
Successor Attorney-in-Fact
At the top of page 2, repeat the agent’s name, then enter the name and address of the alternate agent who would step in if the agent cannot serve.
Date, signature and printed name
On page 2, fill in the day, month and year, then sign on the signature line and print your name beneath it.
Acknowledgment: county, date and officer
Page 3 is the notary acknowledgment. Fill in these lines together with the notary: the county, the date, the officer’s name and title, and the name of the person who appeared.
Officer signature and seal
The notary or other officer signs on the Signature line at the bottom of page 3 and adds the seal.
The paragraph in the middle of page 1 is printed on the form. It lists the actions it covers, such as register, license, sell, transfer ownership, repair, maintain or retitle the vehicle. Read it before you sign.

Print all three pages of the power of attorney template, then sign and print your name on page 2.
Signature and Notarization
The registrant must sign and date the form, stating their title or relationship to the carrier company. This section must be completed in the presence of a notary, who will verify the registrant’s identity and attest to the document’s signing.[2] The notary will then fill in their details, affix their seal, and indicate the type of identification produced by the registrant.[2]
Complete the Declaration of Authorized Agent
On the second page of the form, the agent must acknowledge their understanding and acceptance of the responsibilities and limitations associated with the powers granted. The agent should confirm their familiarity with the IRP and IFTA requirements and the MVC’s procedures. The agent signs and dates this section to validate the POA.
Submission
Once the registrant has signed and dated the POA on page one in front of a notary, and the agent has signed and dated the Declaration of Authorized Agent on page two, file the completed two-page form with the New Jersey Motor Vehicle Commission.[2]

The New Jersey POA and Declaration by Authorized Agent (DAA) is a document that authorizes a representative to handle International Registration Plan (IRP) and International Fuel Tax Agreement (IFTA) matters on behalf of a registrant. Fill Out the Registrant/Licensee Section
Enter the full legal name of the registrant or licensee. This must match the name associated with the IRP and IFTA accounts.[2] Next, provide the Federal Employer Identification Number (FEIN) or Social Security Number (SSN) registered with the IRP and IFTA accounts. Include a contact telephone number and an email address for official communications.
Enter Authorized Agent Information
Specify whether the agent is an individual or a motor carrier service provider company. If the agent is an individual, enter their name and contact information, including telephone number, fax number, and email address. If representing a company, a list of all employees authorized to act as agents must be provided to the Commission, including their names, signatures, driver’s license photocopies, and email addresses.[2] Provide the contact details for the agent or company.
Specify the Acts Authorized by the Registrant/Licensee
Declare the specific powers granted to the agent. This includes the authority to handle IRP and IFTA transactions, access confidential information, and represent the registrant in audit or collection matters. It should be noted that the agent cannot endorse checks, execute compromise consents, or sign documents that require the registrant’s attestation.[2] Attach a photocopy of the registrant’s driver’s license as required.[2]
