DAF Form 104 at a glance
What it is: DAF Form 104 is the Service Medal Award Verification.
Who signs: The persons verifying eligibility fill in the SIGNATURE BLOCK, the SIGNATURE and the DATE.
Award approval authorities use the form to determine qualification for recognition of individuals through an award or a military decoration.

How to fill out DAF Form 104, box by box
SSN, NAME and RANK
Enter the SSN, the NAME as Last, First, Middle Initial, and the RANK.
Providing the SSN is voluntary, but failure to provide the SSN may impede proper placement in the member's military personnel file.
AWARD, UNIT and INCLUSIVE PERIODS
Under UNIT, each time the form is posted, the words "Last Entry" must be included after last item.
Enter the AWARD and the INCLUSIVE PERIODS. For the INCLUSIVE PERIODS, for additional instructions, see AFI 36-2803.
The MEMBER'S INITIAL is optional.
